Insurance Information
We are enrolled in most managed care insurance programs. However, we do not accept Group Health Associates, Anthem Senior Advantage, or health maintenance plans. It is extremely difficult for us to keep track of all the individual
requirements of all the plans available. Your insurance coverage is a contract between you and your insurance provider. Therefore, it is your responsibility to obtain a referral from your primary care physician.

If you do not take the responsibility of obtaining a referral, our facility will have no choice but to bill you directly for services rendered if insurance rejection results from lack of referral.
Surgical Payments and Fees
Under normal circumstances, cosmetic surgery is not covered under insurance plans. Therefore, you may be asked to pay for this type of surgery in advance. The fees for examinations vary depending on the complication of your problem. Additional charges will be applied for diagnostic tests, hearing tests, injections, allergy tests, and special procedures.

Payment Information
Payment for medical service is due at the time of the visit. All co-pays are due at the time of service. If you are unable to pay on the day you receive services from Tri-County E.N.T., please discuss the situation with our office manager. Payment arrangements can be made since our main concern is the betterment of your health. With your cooperation and help, we will be able to concentrate on your treatment and prognosis.
